Form
Introduction
Wholesale pricing should only be available to verified businesses. Registration Forms allow you to collect business details, review applications, and approve qualified customers before granting access. This ensures that only legitimate buyers can view your special offers while keeping the signup process simple and professional.
Form
To create a form, please follow these steps: Form > Create form > Select form types (We are providing a single-step form, the multi-step form will be developed shortly).

Single-step form / B2B registration form
Element
With this feature, you can customise a form that fits your business needs by choosing from built-in fields or adding your own.
To start, you can:
- Name your form title, this title will be displayed internally, your customer won't see the form title
- Name the URL: This is a required step to create the form. Once you enter a URL name, the form will be generated under that URL address, allowing users to access and submit the form through the link.
If a form has been successfully created and the merchant later updates the form URL within the app, the corresponding page URL in Shopify will be updated accordingly. However, the reverse synchronization will not be implemented at this stage, if the page URL is updated in Shopify Admin, it will not automatically update in the app. This will be improved in a future update.

You can add more information fields or custom fields by clicking "Add field" button, and you can edit fields by clicking directly on the fields. Depending on the field type, you will have different configurations.
- Information fields: Customer information, First name, Last name, Email, Password, Company information, Company, Phone, Country & State, Address, City, Zip code, Apartment
- Others: Subcribe to our marketing emails
- Custom fields: Multiple, Radio, Text, Text area, Heading, Number, Date field, Upload files

Behavior
Registration approval
- Manual approval after registration: After customers submit the form, you can go to the list to review and approve the registration if it is qualified.
- Auto-approval after registration: After customers submit the form, customers will be created as customers in Shopify automatically.

- Display success message
- Redirect to login page: This option is disabled if you are selecting the Manual approval option
- Redirect to URL

Moreover, you can apply tags to registered customers after approval, define the tax exemption, and prevent spam by enabling the reCAPTCHA

Apprearance
You can customise your form by changing form style, field size, field style, button style
Form style: Background color, Heading, Heading alignment, Heading format
Field size: Label, Help text, Corner radius
Field style: Label, Help text, Border, Field format
Button style: Font size, Button format, Button alignment, Corner radius, Border thickness, Button background, Button text, Drop shadow, Hover

Publish form
Method 1: Direct URL
Customers access the form via the generated page URL.
Method 2: App Block
Merchants can embed the form in their theme using an app block.
Clicking Open App Embed redirects to the Shopify Theme Editor > Choose the page > Copy the Form ID and Paste > Save

After saving the form successfully, you will be able to see its status, form ID, form name.

Manage list
You can review registration list by clicking "Manage registration list" button.

Registrations are categorized into:
- All
- Pending
- Approved
- Rejected

- When a registration is rejected, the available options are Delete and View.The View option allows you to review the submitted information, but it cannot be edited.
- When a registration is approved, the available options are Edit and Delete.When editing customer information, clicking Save will update the corresponding customer details in Shopify.
After approving the registration, the customer will be created in the Shopify Customer with the filled information.
Note:
- Password field won't be displayed in the registration detail.
- At the moment, due to the limited capacity & resources, we are primarily supporting US market, so it might lead to some errors related to Country / State fields. Please contact our support if this error happens to you
- In the Delete scenario, since the system currently supports only one form, deleting the form will also delete the corresponding Shopify page (even if the page may contain multiple forms). In future versions, when multiple forms are supported, this behavior will be improved accordingly
- The layout / appearance of registration detail might not reflect 100% the storefront layout due to some technical constraints
- New Customer Account: Because
/account/registerdoes not exist, merchants cannot attach the form there. Therefore, password field is not meaningful in New Customer Account, and we are already having improvement plan. - We are improving the Approve/Reject registrations so please contact us if you have any issues or questions related to approve/reject process.
