Add the Withdrawal Form & Button to Your Storefront
Overview
Getting the withdrawal form live on your store takes two things: activating the app embed, then choosing how shoppers reach the form. This guide walks you through both so customers can find and submit their requests with ease.
The withdrawal form is available in all 24 official EU languages, shown automatically in each shopper's language.

Requirement
- A published Shopify theme.
- The Blockify Withdrawal Button app installed on your store.
How to activate the app embed
The app embed loads the withdrawal form on your storefront. It must be on before the button or widget will appear.
Step 1: In your Shopify theme, turn on the Blockify Withdrawal app embed and save.

Step 2: Return to the app Home and confirm the Theme store app embed badge reads On.

How to surface the form for customers
Choose how shoppers reach the form. You can use either option, or both.
A. Use the floating button

B. Add the withdrawal page to a store menu
Use the Add page to store card to place your withdrawal page anywhere in your store's navigation — not just the footer. Your withdrawal page and its link never change; you only choose where customers see it.
Step 1: Go to Settings → Form design and find the Add page to store card, then click Add page.

Step 2: Choose a menu from the Menu dropdown (for example Main menu, Footer, or Catalog), then click Next.

Step 3: Set the Label (default "Withdrawal form") and drag the item into the position you want. The link is read-only and always points to your withdrawal page. Click Add to menu.

Step 4: The card now shows an Active badge and tells you which menu the page is showing in.

To change or remove it later, click Manage menu to switch menus, reorder items, or change the label, or click Remove to take the link out of the menu. Removing only hides it from navigation — your withdrawal page stays live at its link and you can add it back any time. Adding a menu that already contains the withdrawal link won't create a duplicate, and no re-authorization is needed.

Adding the withdrawal form and button to your storefront is part of the core setup and is available on all plans.

- The form or button isn't showing — what should I check?
Confirm the Blockify Withdrawal app embed is on and saved, then refresh the storefront page.
- Do I have to use the floating button?
No. You can use the floating button, a footer menu link, or both. If you'd prefer only a footer link, disable the floating button on the Widget & Form page.
- Where do I change the button's text or style?
On the Widget & Form page — see "Configure the Storefront Widget Button."
