Custom fields for your withdrawal form
Overview
In addition to the built-in fields, you can add your own fields to the withdrawal form — for example a short text box, a dropdown, or a checkbox — to collect the extra information your store needs.

How to add a custom field
Step 1: Open Blockify Withdrawal Form, go to the Settings page, and find the Custom fields card (below Form fields, above Legal settings).

Step 2: Click Add custom field.
Step 3: Enter a Field label, choose a Field type, and toggle Required field if customers must complete it. Add optional Help text to guide them. For a Dropdown, add at least one option.

Step 4: Review the live Form preview, then save the field.
Step 5: Click Save on the Unsaved changes bar to publish your changes to the form.

Field types
You can choose from six field types:
Type | Use it for |

Short text | A single line of text, such as a name or reference. |
Paragraph | Longer, multi-line answers. |
Number | Numeric input. |
Dropdown | A list of options the customer picks from. |
Checkbox | A single yes/no confirmation. |
Date | A date the customer selects. |
Manage your custom fields
- You can add up to 10 custom fields.
- Reorder fields with the up/down arrows to control where they appear on the form.
- Edit or Delete a field at any time.
- Click Discard to revert any unsaved changes.

Where captured values appear
Values customers enter in your custom fields appear on each withdrawal request under Additional details, listed as label: value (checkboxes show Yes/No, dropdowns show the selected option). If you later edit or delete a field, previously captured values still display correctly, because the field's label and type are stored with each request.
Who can use this feature?
Adding custom fields to your withdrawal form is available to all stores using the app.
FAQs
- How many custom fields can I add?
Up to 10.
- When do custom fields appear on the form?
As soon as you click Save on the Unsaved changes bar. Customers then see them on the first step of the form, after the standard fields, in the order you set.
- What happens to captured values if I delete a field later?
Values already captured still display correctly on each request, because the field's label and type are stored with the request.
- Can I make a custom field required?
Yes. Toggle Required field when you add or edit it; required fields are marked with an asterisk (*) on the form.
